MyHealthConnection terms and conditions

MyHealthConnection is a secure, convenient way to access portions of your medical information. With Novant Health UVA Health System Culpeper Medical Center’s patient portal, you can view:

  • Select laboratory results
  • Radiology reports
  • Visit history
  • Discharge instructions
  • Your health summary
  • Immunizations
  • Medical appointments
  • Allergies and conditions

MyHealthConnection is not your official Culpeper Medical Center medical record and may not contain all the information within that record. You can get a copy of your official medical record from our Medical Records Department. You can reach them at 540.829.4386.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

You must be 18 years of age or older to request activation of a MyHealthConnection account.

Electronic communication

Culpeper Medical Center will make its best effort to provide you with a timely response to your email messages. In some cases, the staff member who responds may not be available immediately, so you should allow up to at least three days for a response. Electronic communication should not be used in emergency situations. If you require an immediate response, please do not submit inquiries electronically through this connection. We do not guarantee that MyHealthConnection will be available at all times.

We can only respond to email messages based on the information you provide. If we do not have enough information, we will not be able to provide accurate and reliable services or information.

You will be notified via email when there is new information that you can view in MyHealthConnection. This means that anyone with access to your email will be able to see this notice. Although no private medical information will be sent in the email notice, you may wish to consider this when you provide an email address. If you wish to receive email notices, it is your responsibility to keep your email address up to date in your MyHealthConnection profile.

Proxy Access

If you choose to give proxy access or wish to have proxy access for your child (under 18 years old) these same Terms and Conditions apply when accessing the MyHealthConnection account of another. Registration for proxy account must be done in person through the Medical Records Department. For more information call 540.829.8848.

Security and confidentiality

We afford the same degree of confidentiality to medical information stored in MyHealthConnection as is given to any medical information stored by Culpeper Medical Center. We are committed to protecting the confidentiality of your medical information. We limit Culpeper Medical Center employees’ access and ability to enter or view information based upon their job duties. Firewalls, passwords, encryption and audit trails are used to safeguard your information. We have taken steps to keep all information received electronically as secure as possible against unauthorized access and use.

Your username and password provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyHealthConnection user, your role in maintaining the security of your medical information is to: 1) Change your password on a regular basis, and 2) Keep your login ID and password confidential. The security of your login and password is your responsibility. If you believe your password has been compromised, please change it immediately.

Appropriate Use

Access to MyHealthConnection is granted to give you information about your medical care. Improper use of this system may result in termination of your MyHealthConnection access at the discretion of Culpeper Medical Center.

Termination or deactivation of MyHealthConnection Access

Culpeper Medical Center reserves the right to terminate access to MyHealthConnection at any time and for any reason. The following is a list of reasons for termination of MyHealthConnection access. However, this list is not all-inclusive, and each circumstance would be evaluated individually:

  • Inappropriate use of electronic messaging (excessive use, inappropriate or abusive content)
  • Pending legal issues with Culpeper Medical Center
  • Inappropriate use of MyHealthConnection customer services
  • Other activities that cause a disruption to operations

Automatic deactivation will occur if the number of failed login attempts exceeds established limits.
